The timing of an apparent heart attack is emerging as the central question in the investigation into the death of popular Greek singer Giorgos Mazonakis, 54, judicial and security sources say. Two doctors from a Kolonaki clinic have already been remanded in custody as prosecutors examine how he died after a plasmapheresis procedure.<\/p>\n
According to information obtained by investigators, the initial forensic report does not contain a finding that fully \u201cexplains\u201d the death. During the autopsy, however, doctors observed ischemia of the interventricular myocardium extending toward the apex of the heart. Histological tests are expected to confirm or rule out that finding.<\/p>\n
The interventricular septum is the muscular wall between the heart\u2019s two ventricles. Ischemic damage described as extending toward the apex indicates a geographic pattern of inadequate blood and oxygen supply. The key issue is whether the ischemia lasted long enough to cause irreversible death of heart-muscle cells \u2014 a fatal myocardial infarction \u2014 and how long before death the damage began. Pathologists will look for cellular changes, analysis of myocardial fibers, and the presence of neutrophils to help date the onset.<\/p>\n
Mazonakis had appeared \u201cvery good\u201d to a taxi driver who took him to the clinic and to a pharmacist in the southern suburbs who saw him shortly beforehand. Investigators are also examining reports that he fainted in a shop in Nea Erythraia three days before he died at the clinic on Karneadou Street.<\/p>\n
The forensic examination did not find cardiomegaly or hypertrophy of the heart. Examiners are weighing the macroscopic appearance of the heart, histology, the condition of the coronary arteries, any prior scars or infarcts, and the singer\u2019s medical history and symptoms before death.<\/p>\n
Linking the death directly to the plasmapheresis procedure itself is expected to be difficult. Autopsy findings so far showed no thrombosis that could be attributed to the treatment. Histology is also considered unlikely to demonstrate air bubbles or swelling in vital organs that would prove a fatal air embolism or a large transfusion-related event.<\/p>\n
Prosecutors are also looking at other alleged failings: use of unauthorized equipment at the clinic, delayed notification of emergency medical services, an initial attempt to downplay the singer\u2019s condition, and whether toxicology will detect benzodiazepines or other drugs that may have been given to facilitate the procedure.<\/p>\n
Further tests \u2014 myocardial histology, quantitative toxicology, and correlation of machine data with vital signs \u2014 will determine whether the ischemia began hours earlier or in the minutes around the procedure. Until those results are in, investigators say the precise sequence of events remains open.<\/p>\n
